no, but i bet if you went to your states D.O.T. website you can find the answer to that

Its probably a form you have to sign stating that you know your license is suspended, Im just guessing though...and I think you have to give them your actual license

In MN they send you a letter, you don't have to actually hand over your I'd

The form is probably for a probationary license for work or school

Suspended = no driving

Usually, you have to wait a certain amount of time before they'll review your case and then they still might not give you a probationary license
